If it was a beta it could have been a CD version of Sonic 1,like there was supposed to be a CD version of Sonic 2,but I doubt it.

Sonic 2 CD? Yeah, that was like Sonic 2 with added levels and features, right? I read an interview about it awhile ago. Maybe they considered doing the same for Sonic 1, but got canned in conceptual stages...

Or perhaps Sonic CD was originally supposed to be just an enhanced version of Sonic 1.

Looks a bit fake to me as that's basically the sonic 1 box art but just edited slightly to make it look as if its a real mega cd game

It looks fake, but it's real. The original image was a very small and blurry GIF, that's why it looks so bad.
