One more bug - managed to end Aquatic Ruin Zone 1 as immobile Sonic (took the top route and was spinning when I became immobile, so span all the way to the end sign and hit it). As Sonic couldn't move off-screen, the act never ended, but since the end sign had been hit Sonic never died either. Might be worth adding a check to see if Sonic is immobile and allowing the act to end (or Sonic to die, if you're feeling mean) in this situation.
Still hilarious seeing him sitting there endlessly in his obese state, though. XD

There was this one educational flash game that did just this. It was a HUGE ripoff of Sonic (springs, loops, spikes, fast paced level design) that when you eat unhealthy food you got slower and fatter. Music changed too, it became all slow and "wobbily" sounding. However, that game allowed you to stop and do pushups to get yourself back to a skinny state.
It used to be here, but now seems to be currently 404'd, sadly.

It does that sometimes even if you're in ball form.
EDIT: I think I figured out why it happens... I think one of the boosters forces you out of ball form... and if you are too big already, you're dead because you can't go back into it.
