I am bumping this topic with a different issue for Sonic R on the PC.
I decided to play it recently, and the game works, but the backgrounds and titles are all flashing quite quickly like a strobe effect, and I don't know why.
Playing on an XP Media Center Edition 2005. The compatibility settings are just for 256 colors, the rest seem to not let the game work at all. I also didn't have music until I restarted the computer the first time.

What are your PCs specs? Sonic R usually worked on XP for me until I upgraded my computer (Sonic R really hates dual-core processors, apparently). Unless XP Media Center Edition is drastically different from regular XP in some way I'm not aware of, your problems are probably being caused by hardware incompatibilities.
